Statement of Qualifications This form is to be completed by an officer of your organization and returned to Servant Solutions. Be sure to attach the requested supporting documents: 501(c)(3) verification and a statement of beliefs.
Eligibility and Participation Schedule This form details how the retirement plan will be administered at your organization. It must be approved by your organization’s board and signed by an authorized officer of the organization.
Salary Reduction Agreement Employers and employees can specify and sign for pre-tax contributions to the Servant Solutions Retirement Plan here. This form outlines and serves as a salary reduction agreement.
Remittance Form This form communicates to us where to direct account contributions as well as which contribution type is preferred.
