Below are the forms you need to complete to join the Servant Solutions Retirement Plan (be sure your employer has enrolled with Servant Solutions prior to completing these forms!). If you choose to roll funds into your Servant Solutions account, please complete the below rollover form. Membership Application You can access our Retirement Plan Membership Application here, where you’ll be able to select from various investment options and find additional information and details about the Servant Solutions Retirement Plan.
Beneficiary Form Being fully prepared for the future is important, and it’s smart to create a plan for your finances now. Fill out this form to designate any beneficiaries of your retirement plan benefits in the event you pass away before receiving them all.
Rollover Form This form initiates funds to be directly rolled over to the Servant Solutions Retirement Plan. It also provides information and details specifying which contributions are/are not eligible for direct rollover.
Salary Reduction Agreement Employers and employees can specify and sign for pre-tax contributions to the Servant Solutions Retirement Plan here. This form outlines and serves as a salary reduction agreement.
Update Contact and/or Marriage Information This form will allow you to update data files maintained by Servant Solutions for your retirement account. Please complete this form to let us know of any changes to your account (print and write legibly) and return via fax or our secure document upload portal. Thank you.
